Substance Use Disorder Rates/Covered Benefit Revisions

Effective for dates of services on and after October 1, 2015, the Substance Use Disorder (SUD) rates, units and procedure codes will be established per the attachment to this policy. Provider types and specialties allowed for each service are also notated in the attachment for this policy.
