Georgia Care Coordination
Care Coordinators help in a special way at Cenpatico. Care coordinators do the following to help our members:
- Call and send letters to members after they leave the hospital to make sure they get appointments for counseling and medication
- Call members after they leave the hospital to find out how they are doing and help them find an appointment
- Contact parents of children that have been in the hospital more than one time and help them with problems they may have such as getting to appointments and trouble with a doctor
- Find counselors or doctors and help find emergency appointments
- Take calls from Peach State Health Plan about members who need behavioral health services
Help find special doctors, like a counselor who speaks a foreign language
